Media Release For Social Media

I, {{fullname}}, residing at {{address}}, hereby grant {{company_name}} (the “Company”), its representatives, and employees, permission to use my likeness, image, voice, and/or statements in photographs, videos, audio recordings, or any other media (hereinafter collectively referred to as “Media”) for promotional, marketing, and educational purposes related to the Company’s social media activities.

Voluntary Consent

I understand and agree that by signing this form, I am voluntarily giving my consent for the Company to capture and use my Media on its official social media channels, including but not limited to Facebook, Instagram, Twitter, LinkedIn, YouTube, and TikTok. I understand that the Media may be used in various formats and contexts, including posts, stories, reels, live streams, and advertisements.

Usage and Purpose

I authorize the Company to use, reproduce, publish, distribute, and display the Media in whole or in part, without restriction or limitation, in perpetuity, throughout the world, in any and all media now known or hereafter developed. The purpose of such use may include, but is not limited to, promoting the Company’s products, services, events, and brand.

I confirm that the Company will not be obliged to pay me any royalties, fees, or other compensation in connection with the use of the Media.

Waiver and Release

I hereby release and discharge the Company, its directors, officers, employees, agents, and assigns from any and all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I waive any right to inspect or approve the finished product where my Media appears. I understand that I will not receive any financial compensation for the use of my Media.

Representations and Warranties

If the individual is a minor (under 18), a parent or legal guardian must sign this form below.
