1. Introduction and Purpose

We recognize that employees may experience personal or work-related challenges that can impact their well-being, job performance, and overall quality of life. To address these needs, {{company_name}} offers a confidential Employee Assistance Program (EAP).

The purpose of this policy is to outline the scope, benefits, and procedures of our EAP, ensuring that employees have access to professional counseling and support services when needed.

2. What is the Employee Assistance Program (EAP)?

The EAP is a confidential, professional counseling and referral service designed to help employees and their immediate family members address a wide range of personal and work-related issues. These issues may include, but are not limited to, stress, anxiety, depression, grief, marital and family problems, substance abuse, financial difficulties, legal concerns, and workplace conflicts.

3. Eligibility

All full-time and part-time employees of {{company_name}} are eligible to utilize the EAP. In addition, immediate family members (spouse/partner and dependent children) of eligible employees are also covered under the program.

4. Confidentiality

Confidentiality is a cornerstone of the EAP. All interactions between employees, their family members, and the EAP provider are strictly confidential, within the limits of the law. No information about an employee's use of the EAP will be shared with {{company_name}} without the explicit written consent of the employee, except in cases where there is a legal requirement to do so (e.g., threat of harm to self or others).

The EAP provider maintains separate records from {{company_name}} and these records are not accessible by company management or HR personnel.

5. Services Offered

The EAP offers a variety of services, which may include:

a. Short-term counseling for personal and work-related issues.

b. Referrals to specialized community resources for long-term support, legal and financial advice, and child/elder care resources.

c. Crisis intervention and critical incident stress management.

d. Educational materials and workshops on various well-being topics.

6. Accessing the EAP

Employees can access the EAP directly by contacting the EAP provider at {{eap_provider_phone_number}} or visiting their website at {{eap_provider_website}}. Services are available {{eap_hours_of_operation}}.

Management may also suggest employees consider utilizing the EAP, but the decision to seek assistance remains entirely with the employee.

7. Cost of Services

{{company_name}} covers the cost of a specified number of EAP sessions per eligible individual per issue per year, as determined by our contract with the EAP provider. Any additional sessions or referrals to external services may be the financial responsibility of the employee, subject to their individual insurance coverage.
