AFFIDAVIT OF EXECUTION

I, the undersigned, {{deponent_name}} (Identity Number: {{deponent_id_number}}), do hereby make oath and state:

1. IDENTIFICATION

I am an adult person residing at {{deponent_address}}.

2. PURPOSE OF AFFIDAVIT

This affidavit relates to the execution of the following document (hereinafter referred to as “the Document”):

3. EXECUTION OF DOCUMENT

I have no objection to taking the prescribed oath/affirmation.
