Any contravention of these rules may result in their removal from the activity/event, and I agree to arrange for their immediate collection.

Waiver of Liability

I hereby release, indemnify, and hold harmless {{company_name}}, its officers, employees, agents, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child or to any property belonging to my child, while participating in the activity/event, unless due to gross negligence or willful misconduct of {{company_name}}.
