Employee Photo and Recording Release Form

I, {{employee_name}}, residing at {{employee_address}}, and employed by {{company_name}}, hereby irrevocably authorize and grant permission to {{company_name}}, its subsidiaries, affiliates, and their respective agents, employees, licensees, and assigns (collectively, the "Company") to photograph, videotape, and/or record my image, likeness, voice, and quotes (collectively, "Recordings").

Purpose of Use

I understand and agree that these Recordings may be used for various legitimate business purposes including, but not limited to, internal communications, external promotional materials, marketing campaigns, public relations activities, training programs, company websites, social media platforms, presentations, and other corporate communications channels. I agree that the Company may use these Recordings in any media now known or hereafter developed, including print, digital, and electronic formats.

Waiver of Review and Approval

I waive any right to inspect or approve the finished product where my image, likeness, voice, or quotes appear. I also waive any right to royalties or other compensation arising or related to the use of the Recordings.

Release of Liability

I hereby release and discharge the Company, its officers, directors, employees, and agents from any and all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. This release includes, but is not limited to, claims for libel, defamation, invasion of privacy, or infringement of any right of publicity.

Term of Release

This release is perpetual and applies to all Recordings made during my employment with the Company or at events related to my employment.

Voluntary Consent

I affirm that my participation and consent to this release is voluntary and that I have not been coerced or unduly influenced into signing this document. I have been given the opportunity to ask questions regarding this release and fully understand its terms.

Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the jurisdiction in which {{company_name}} is primarily registered.
