1. Purpose
The purpose of this policy is to establish a systematic approach for investigating all workplace incidents, accidents, and near misses. This includes identifying root causes, developing and implementing corrective and preventative actions, and improving overall health and safety performance. This policy aims to minimise recurrence, protect employees and company assets, and comply with all relevant legal and regulatory requirements in Southern Africa.
2. Scope
This policy applies to all employees, contractors, visitors, and any other persons present on company premises or engaged in company-related activities. It covers all incidents, including injuries, occupational diseases, property damage, environmental incidents, and near misses, regardless of severity.
3. Definitions
**Incident:** An undesired event that resulted in or could have resulted in harm to people, damage to property, or loss to process.
**Accident:** An incident that results in injury, illness, or property damage.
**Near Miss:** An unplanned event that did not result in injury, illness, or damage – but had the potential to do so.
**Root Cause:** The fundamental reason(s) for an incident, which if eliminated, would prevent recurrence.
**Investigation Team:** A group of individuals assigned to professionally and thoroughly investigate an incident, comprising relevant personnel such as safety officers, supervisors, and employee representatives.
4. Responsibilities
**Management:** Ensure adequate resources are provided for incident investigations, promote a culture of reporting, and ensure implementation of corrective actions.
**Supervisors:** Initiate immediate incident response, ensure safety of the scene, report incidents promptly, participate in investigations, and implement corrective actions within their area of control.
**Employees:** Report all incidents and near misses immediately, cooperate with investigations, and comply with all safety procedures.
**Health and Safety Representative/Officer:** Lead incident investigations, provide expertise, ensure compliance with this policy and relevant legislation, and maintain incident records.
5. Incident Reporting Procedure
5.1. **Immediate Reporting:** All employees must report any incident or near-miss to their immediate supervisor or designated safety personnel as soon as practically possible, but no later than {{reporting_timeframe}} (e.g., 24 hours) of its occurrence.
5.2. **Reporting Mechanism:** Reports should be made using the designated {{incident_report_form}} or via {{reporting_method}}.
5.3. **Information to Provide:** The report should include the date, time, location, persons involved, a brief description of the incident, and any immediate actions taken.
6. Incident Investigation Procedure
6.1. **Scene Preservation:** The incident scene must be preserved until the investigation team has completed their initial assessment, unless doing so poses further risk.
6.2. **Investigation Team Formation:** For significant incidents, an investigation team will be formed, typically including the Supervisor, Safety Officer, and a relevant employee representative.
6.3. **Data Collection:** The investigation will involve collecting evidence such as witness statements, photographs, video footage, relevant documents (e.g., maintenance logs, risk assessments), and physical evidence.
6.4. **Analysis:** The team will analyse all collected data to determine the sequence of events, immediate causes, and underlying root causes of the incident using techniques such as 5 Whys, Fishbone diagrams, or Fault Tree Analysis.
6.5. **Investigation Report:** A comprehensive investigation report will be compiled within {{report_completion_time_frame}} (e.g., 5 working days) of the incident. This report will detail the findings, root causes, and recommended corrective and preventative actions.
6.6. **Communication of Findings:** Relevant findings and recommendations will be communicated to affected parties and management.
7. Corrective and Preventative Actions (CAPA)
7.1. **Action Plan Development:** Based on the investigation report, a CAPA plan will be developed, assigning specific actions, responsibilities, and completion deadlines.
7.2. **Implementation:** Assigned individuals are responsible for implementing their respective actions.
7.3. **Follow-up and Verification:** The Safety Officer or designated personnel will follow up to ensure that all corrective and preventative actions have been effectively implemented and are achieving the desired outcomes.
7.4. **Review:** The effectiveness of the CAPA plan will be periodically reviewed.
8. Training and Awareness
All employees will receive appropriate training on this Incident Investigation Policy, including their reporting responsibilities and the importance of incident prevention. Specific training on investigation techniques will be provided to supervisors and designated investigation team members.
9. Record Keeping
All incident reports, investigation findings, CAPA plans, and related documentation will be securely maintained for a minimum period of {{record_keeping_period}} (e.g., 5 years) in accordance with legal requirements and company policy. 10. Policy Review
