Multimedia Publicity - Privacy Release

I, {{full_name}}, residing at {{home_address}}, hereby grant {{company_name}} (hereinafter referred to as 'the Company'), its representatives, employees, and assigns, permission to use, reproduce, and distribute my image, likeness, voice, and/or statements in photographs, videos, audio recordings, or any other multimedia format.

Purpose of Use

I understand that the multimedia content may be used for promotional, publicity, educational, or informational purposes related to the Company's activities. This may include, but is not limited to, use in brochures, websites, social media, advertisements, press releases, presentations, and internal communications.

Scope of Release

This release includes permission to copyright, publish, or sell the multimedia content in any manner the Company deems appropriate. I waive any right to inspect or approve the finished product, including written copy, where my image, likeness, voice, or statements appear.

Compensation and Acknowledgment

I understand that I will not receive any financial compensation for the use of my image, likeness, voice, or statements, and I acknowledge that such use is solely for the benefit of the Company. I further understand that I will not be acknowledged as having provided my image, likeness, voice, or statements unless specifically agreed upon in writing.

Term of Release

This release is perpetual and worldwide, meaning the Company may use the multimedia content indefinitely and in any geographic location, unless otherwise revoked in writing by me.

Revocation of Consent

I understand that I have the right to revoke this consent at any time by providing written notice to {{company_name}} at {{company_email_for_revocation}}. Upon receipt of such notice, the Company will make reasonable efforts to discontinue future use of the multimedia content, though content already published or distributed prior to revocation may remain in circulation.

Representations and Warranties
