1. Introduction and Policy Statement

{{company_name}} is committed to providing a safe and healthy working environment for all its employees, contractors, visitors, and stakeholders. This policy establishes the framework for reporting all safety incidents, near-misses, and hazards, and for conducting thorough investigations to prevent recurrence. Our goal is to eliminate all workplace injuries and illnesses through proactive measures and continuous improvement.

All employees are expected to comply with this policy and report any safety concerns promptly. Management is responsible for providing the necessary resources for incident investigation and for implementing effective corrective actions.

2. Scope

This policy applies to all operations, facilities, and activities conducted by or on behalf of {{company_name}}. It covers all employees (full-time, part-time, temporary), contractors, visitors, and any other persons present on company premises or engaged in company-related work.

3. Definitions

**Incident:** An event or series of events that has resulted in harm (injury, illness, or damage to property) or has the potential to result in harm.

**Near-Miss:** An unplanned event that did not result in injury, illness, or damage – but had the potential to do so.

**Hazard:** A source or situation with a potential for harm in terms of human injury or ill-health, damage to property, damage to the environment, or a combination of these.

**Investigation:** A systematic process to determine the root causes of an incident or near-miss and to identify corrective and preventive actions.

4. Reporting Procedures

**4.1 Immediate Actions:** In the event of an incident resulting in injury or significant property damage, immediate first aid or emergency services should be sought. The area should be secured to prevent further incidents.

**4.2 Reporting Timeline:** All incidents, near-misses, and hazards, regardless of severity, must be reported to a supervisor or designated safety officer within **{{reporting_timeframe}}** of their occurrence or discovery.

**4.3 Reporting Method:** Reports shall be made using the official Incident/Near-Miss Report Form (Form-{{form_number}}), available at {{form_location}}. Completed forms should be submitted to {{department_for_submission}}.

**4.4 Information Required:** The report should include, but is not limited to: date, time, and location of the incident/near-miss; names of individuals involved or witnesses; a detailed description of what happened; any observed causes; and immediate actions taken.

5. Incident Investigation Process

**5.1 Investigation Team:** An investigation team, typically comprising a supervisor, safety officer, and relevant employees/specialists, will be assigned within **{{investigation_team_assignment_timeframe}}** of a reported incident. The size and composition of the team will depend on the severity and complexity of the incident.

**5.2 Data Collection:** The investigation team will gather all relevant information including witness statements, photographs, video footage, maintenance records, training records, and work procedures.

**5.3 Root Cause Analysis:** The team will conduct a thorough root cause analysis to identify the underlying systemic failures that contributed to the incident, rather than focusing solely on immediate causes or individual error.

**5.4 Investigation Report:** A detailed Incident Investigation Report (Report-{{report_number}}) will be prepared, outlining the findings, identified root causes, and recommended corrective and preventive actions. This report will be completed within **{{investigation_report_completion_timeframe}}** of the incident.

6. Corrective and Preventive Actions (CAPA)

**6.1 Development of CAPA:** Based on the investigation report, appropriate corrective and preventive actions will be developed. Corrective actions aim to eliminate the cause of a detected nonconformity or other undesirable situation. Preventive actions aim to eliminate the cause of a potential nonconformity or other undesirable potential situation.

**6.2 Implementation:** CAPAs will be assigned to responsible individuals/departments with specified completion dates. These actions may include changes to procedures, equipment modifications, additional training, or disciplinary action where appropriate.

**6.3 Follow-up and Verification:** The safety officer or designated manager will verify the effective implementation of all CAPAs and monitor their effectiveness in preventing recurrence. This may involve follow-up inspections or audits.

7. Training and Communication

All employees will receive training on this Safety Reporting and Incident Investigation Policy during their induction and at regular intervals (e.g., annually). Key findings from incident investigations and implemented CAPAs will be communicated to relevant employees to foster a culture of continuous learning and safety awareness.

8. Record Keeping

All incident reports, investigation reports, CAPA logs, and related documentation will be securely maintained for a minimum of **{{record_retention_period}}** in the {{record_storage_location}} department. These records will be accessible for audit and review purposes.

9. Review and Improvement

This policy and its associated procedures will be reviewed annually, or as necessitated by changes in legislation, company operations, or incident trends, by the {{review_department}} to ensure its continued suitability, adequacy, and effectiveness. Recommendations for improvement will be incorporated.
