AFFIDAVIT OF EXECUTION

I, {{affiant_name}} (Identity Number: {{affiant_id_number}}), an adult residing at {{affiant_address}}, do hereby make oath and state:

IDENTIFICATION OF DOCUMENT

1. I refer to a document titled "{{document_title}}" (hereinafter referred to as "the Document"), dated {{document_date}}.

EXECUTION OF DOCUMENT

2. The Document was signed in my presence by the Testator/Signatory, {{signatory_name}} (Identity Number: {{signatory_id_number}}), on {{signing_date}}, at {{signing_location}}.

3. The Testator/Signatory appeared to be of sound mind and memory and free from duress or undue influence at the time of signing.

4. I attest that the signature appearing on the Document is indeed the true signature of the Testator/Signatory.

WITNESS ATTESTATION

5. I further attest that I, along with {{witness_name_2}} (Identity Number: {{witness_id_number_2}}), were present together and witnessed the signing of the Document by the Testator/Signatory.

7. This Affidavit is made for the purpose of providing sworn evidence of the proper and lawful execution of the Document.
