Bonus Agreement

This Bonus Agreement ("Agreement") is made and entered into on this the {{day}} day of {{month}}, {{year}}, by and between {{company_name}} (hereinafter referred to as "the Company") and {{employee_name}} (hereinafter referred to as "the Employee").

Purpose of Bonus

Eligibility Criteria

To be eligible for a bonus, the Employee must:

a) Be actively employed by the Company on the date of bonus payout.
