Agreement to Convert Employment Status

This Conversion Agreement (hereinafter "Agreement") is entered into on {{date}}, by and between {{company_name}} (hereinafter "the Company"), with its principal place of business at {{company_address}}, and {{employee_name}} (hereinafter "the Employee"), residing at {{employee_address}}.

WHEREAS, the Employee has been employed by the Company in the capacity of {{previous_role_title}} under a {{previous_employment_type}} employment agreement commencing on {{previous_start_date}};

WHEREAS, the Company and the Employee mutually agree to convert the Employee's employment status from {{previous_employment_type}} to {{new_employment_type}} (hereinafter "the New Employment Status") effective from {{conversion_effective_date}};

NOW, THEREFORE, in consideration of the mutual covenants and agreements contained herein, the parties agree as follows:

Terms and Conditions of New Employment Status

5.1. The Employee's new role title shall be {{new_role_title}}.

5.2. The Employee's annual gross salary shall be {{new_salary_amount}} {{currency_symbol}} ({{currency_word_form}}), payable in {{payment_frequency}} installments, subject to standard deductions as required by law.

5.3. The Employee shall be eligible for benefits as per the Company's policy for {{new_employment_type}} employees, including but not limited to health insurance, {{other_benefits_details}}.

5.4. The Employee's working hours shall be from {{start_time}} to {{end_time}}, {{working_days_per_week}} days a week, {{total_working_hours_per_week}} hours per week, with {{break_time}} break time, from {{start_day}} to {{end_day}}.

5.5. The probationary period for the New Employment Status shall be {{probationary_period_duration}} commencing from {{conversion_effective_date}}. During this period, employment may be terminated by either party with {{probationary_notice_period}} notice.

5.6. All other terms and conditions of employment, including confidentiality clauses, intellectual property rights, and codes of conduct, as outlined in the Company's Employee Handbook and previous agreements not expressly amended herein, shall remain in full force and effect.

5.7. The Employee acknowledges receipt and understanding of the Company's relevant policies and procedures applicable to their new employment status.

Severability

If any provision of this Agreement is found to be invalid or unenforceable, such provision shall be severed from this Agreement, and the remainder of the Agreement shall continue in full force and effect.

Governing Law

This Agreement shall be governed by and construed in accordance with the laws of {{country}}.

Entire Agreement

This Agreement constitutes the entire understanding between the Company and the Employee concerning the conversion of employment status and supersedes all prior discussions, negotiations, and agreements, whether oral or written, relating thereto.

Acceptance of Agreement
