1. Introduction and Purpose

This Paid Time Off (PTO) Policy sets forth the guidelines and procedures for paid time off for eligible employees of {{company_name}}. The purpose of this policy is to provide employees with time away from work for rest, personal appointments, illness, and other reasonable needs, while ensuring business continuity and fairness.

This policy applies to all full-time employees who have completed their probationary period.

2. PTO Accrual

Employees will accrue PTO at a rate of {{accrual_rate}} hours per {{pay_period}} (e.g., eight hours per month, or 120 hours per year). PTO accrual begins on the first day of employment, but can only be utilized after successful completion of the probationary period.

The maximum amount of PTO that can be accrued and carried over to the next year is {{maximum_carry_over_hours}} hours. Any unused PTO exceeding this limit at the end of the calendar year will be forfeited.

3. Requesting PTO

Employees must submit a PTO request through the designated HR system or to their direct supervisor at least {{notice_period_days}} days in advance for planned leave. In cases of unforeseen circumstances or emergencies, employees should notify their supervisor as soon as practically possible.

All PTO requests are subject to management approval, taking into consideration business operational requirements and staffing levels. Approval is not guaranteed and may be denied if it would disrupt critical business functions.

4. Use of PTO

PTO can be used for various reasons, including but not limited to: vacation, personal appointments, family emergencies, and short-term illness. For extended illness, separate sick leave policies may apply.

Employees are responsible for managing their accrued PTO. PTO cannot be used in advance of its accrual.

5. PTO During Termination of Employment

Upon termination of employment, employees will be paid out for any unused, accrued PTO up to a maximum of {{payout_limit_hours}} hours, in accordance with applicable labor laws and internal company policy.

In cases of gross misconduct or immediate dismissal, the company reserves the right not to pay out unused PTO, in compliance with country-specific labor legislation.

6. Holiday and Weekend PTO

When a public holiday falls during an approved PTO period, the public holiday will not be counted as a PTO day and employees will be paid for the holiday as per normal company policy.

Weekends and regularly scheduled days off that fall within a PTO period will not be counted as PTO days unless specified otherwise or if the employee is part of a special shift arrangement.

7. Policy Review

This policy will be reviewed periodically, at least once every {{review_period_years}} years, and may be amended at the discretion of {{company_name}} management. Employees will be notified of any significant changes to this policy.
