In-Home Care Agreement

This In-Home Care Agreement ("Agreement") is made and entered into on this {{day}} day of {{month}}, {{year}}, by and between:

Hereinafter referred to as the "Client",

Hereinafter referred to as the "Caregiver".

Recitals

WHEREAS, the Client desires to engage the Caregiver to provide in-home care services;

WHEREAS, the Caregiver is qualified and willing to provide such services under the terms and conditions set forth herein;

NOW, THEREFORE, in consideration of the mutual covenants and promises herein contained, the parties agree as follows:

Services to be Provided

The Caregiver agrees to provide the following in-home care services to the Client:

The specific schedule of services will be agreed upon by both parties and may be attached as Schedule A to this Agreement. Any changes to the services or schedule must be mutually agreed upon in writing.

Term of Agreement

This Agreement shall commence on {{start_date}} and shall continue until {{end_date}} or until terminated earlier in accordance with the provisions of this Agreement.

This Agreement may be extended by mutual written agreement of both parties.

Compensation

The Client agrees to pay the Caregiver a rate of {{hourly_rate}} per hour / {{daily_rate}} per day / {{weekly_rate}} per week / {{monthly_rate}} per month for services rendered.

Reimbursement for mutually agreed-upon expenses incurred by the Caregiver on behalf of the Client will be provided upon presentation of valid receipts. Such expenses shall not exceed {{expense_limit_per_period}} without prior written approval.

Responsibilities of the Caregiver

The Caregiver agrees to:

a) Perform all services with diligence, professionalism, and respect for the Client's privacy and dignity.

b) Maintain confidentiality of all Client information.

c) Notify the Client promptly of any issues or concerns regarding the Client's health or well-being.

d) Adhere to the agreed-upon schedule and provide timely and reliable care.

e) Not bring any unauthorised persons into the Client's home.

Responsibilities of the Client

The Client agrees to:

a) Provide a safe and suitable environment for the Caregiver to perform their duties.

b) Provide accurate and complete information regarding their health status and care needs.

c) Provide clear instructions regarding the services to be rendered.

d) Make timely payments as agreed upon in this Agreement.

Termination

This Agreement may be terminated by either party with {{notice_period_days}} days' written notice.

Either party may terminate this Agreement immediately in the event of a material breach of its terms by the other party.

Governing Law

This Agreement shall be governed by and construed in accordance with the laws of {{country_name}}.

Entire Agreement

This Agreement constitutes the entire agreement between the Client and the Caregiver and supersedes all prior discussions, negotiations, and agreements, whether oral or written.
