RELEASE OF LIABILITY WAIVER

This Release of Liability Waiver (hereinafter referred to as the "Agreement") is made and entered into on this {{day}} day of {{month}}, {{year}}, by and between:

AND

**{{released_party_name}}** (hereinafter referred to as the "Releasee"), with its principal place of business at {{released_party_address}}.

RECITALS

WHEREAS, the Releasor desires to participate in the activity of {{activity_description}} (hereinafter referred to as the "Activity") organized or facilitated by the Releasee.

WHEREAS, the Releasor understands and acknowledges that the Activity involves inherent risks, hazards, and dangers, including but not limited to those described herein.

WHEREAS, the Releasor, being fully aware of the risks involved, freely and voluntarily chooses to participate in the Activity.

NOW, THEREFORE, in consideration of the mutual covenants contained herein and for other good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, the parties agree as follows:

ASSUMPTION OF RISK

The Releasor acknowledges that participation in the Activity involves certain inherent risks, including but not limited to: {{list_of_risks_and_hazards}}. The Releasor fully understands and voluntarily accepts these risks, and acknowledges that injuries, damages, or losses may occur as a result of participation in the Activity.

RELEASE AND WAIVER OF LIABILITY

The Releasor hereby releases, waives, discharges, and covenants not to sue the Releasee, its officers, directors, employees, agents, representatives, and affiliates (hereinafter collectively referred to as the "Released Parties") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the Releasor, or to any property belonging to the Releasor, whether caused by the negligence of the Released Parties or otherwise, while participating in the Activity, or while in, on, or upon the premises where the Activity is being conducted.

INDEMNIFICATION

The Releasor further agrees to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs, including reasonable attorney’s fees, that they may incur due to the Releasor’s participation in the Activity, whether caused by the negligence of the Releasor or otherwise.

MEDICAL TREATMENT

The Releasor authorizes the Releasee to obtain medical care for them in the event of an emergency during the Activity, at the Releasor's expense. The Releasor also confirms that they have {{medical_insurance_provider}} insurance policy number {{medical_insurance_policy_number}}.

GOVERNING LAW

This Agreement shall be governed by and construed in accordance with the laws of {{country}}, without regard to its conflict of laws principles.

SEVERABILITY

If any provision of this Agreement is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

ENTIRE AGREEMENT

This Agreement constitutes the entire agreement between the Releasor and the Releasee and supersedes all prior agreements, understandings, and negotiations, whether written or oral, relating to the subject matter hereof.

ACKNOWLEDGMENT OF UNDERSTANDING

The Releasor acknowledges that they have read this Release of Liability Waiver, fully understand its terms, and understand that they are giving up substantial rights, including their right to sue. The Releasor acknowledges that they are signing this Agreement freely and voluntarily, and intend their signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
