Testator Information

Appointment of Executor

I hereby nominate and appoint {{executor_full_name}}, residing at {{executor_address}}, as the Executor of this my Last Will and Testament. If the aforementioned Executor is unable or unwilling to serve, or ceases to serve for any reason, I then nominate and appoint {{alternate_executor_full_name}}, residing at {{alternate_executor_address}}, as alternate Executor.

Appointment of Guardian for Minor Children

Payment of Debts and Taxes
