SUBJECT: EXTENSION OF AGREEMENT NO. {{agreement_number}}

This Extension of Agreement (hereinafter referred to as 'this Extension') is made and entered into on this {{day}} day of {{month}}, {{year}}, by and between:

AND

Party A and Party B are hereinafter collectively referred to as 'the Parties' and individually as 'Party'.

RECITALS

WHEREAS, the Parties previously entered into an agreement titled '{{original_agreement_title}}', dated {{original_agreement_date}} (hereinafter referred to as 'the Original Agreement'), concerning {{brief_description_of_original_agreement}}.

WHEREAS, the term of the Original Agreement is set to expire on {{original_expiry_date}}.

WHEREAS, the Parties desire to extend the term of the Original Agreement under the terms and conditions set forth herein.

AGREEMENT TO EXTEND

The Parties hereby agree to extend the term of the Original Agreement for an additional period commencing on {{new_start_date}} and terminating on {{new_expiry_date}} (hereinafter referred to as 'the Extended Term').

All terms and conditions of the Original Agreement, except as expressly modified by this Extension, shall remain in full force and effect during the Extended Term.
